Submitting a Claim for Reimbursement
If an out-of-network provider does not accept insurance billing, you can submit a claim for reimbursement under your Enloe Health plan by doing one of the following:
- Upload the claim on myhnas.com or
- Email them to hnas.enloe@hnas.com
When submitting a claim, it is important that all necessary information is provided. This includes:
- Member name and identification number
- Date of service
- Procedure code
- Diagnosis code
- Billed amount
- Provider name
- Member address
- Member phone number
- Member tax identification number
