We are pleased to offer a robust benefits package to our employees. Use the links below to find benefit documents, forms and company information.
For a hard copy of this same information, stop by or call the Human Resources office at 530-332-7344.
To get your Explanation of Benefits (EOB), call the insurance companies and request new hard copies, or log on to their websites to print them.
To get prescription receipts, you can go to your pharmacy and request a summary printout for the year. Or you can log on to the pharmacy company website and print it from there. Visit our Pharmacy Benefits page for the website link and more information.
Form 1095 is the new IRS tax form required by the Affordable Care Act. You will need to keep this form for your records. It contains benefits-related information, and it is mailed to employees at the end of January.
